Care Coordinator | Up to £28,000 + Bonus Scheme | Monday–Friday + Oncall duties
Our client, a compassionate and well-established domiciliary care provider, is seeking a Care Coordinator to join their dedicated and growing team. As a Care Coordinator, you will play a key role in ensuring the smooth running of day-to-day care operations. You’ll work closely with the Registered Manager, care team, and families to coordinate and manage care schedules, support compliance, and help deliver safe, reliable, and compassionate care to every client.
What We’re Looking For:
A Level 3 Health and Social Care qualification
Experience as a Care Coordinator
Excellent organisational and time management skills
Strong communication and problem-solving abilities
Confident using care management systems and IT tools (e.g., rota systems, databases)
A full UK driving licence and access to a vehicle is essential
A genuine passion for providing high-quality, person-centred care

Key Responsibilities:
Schedule and coordinate care staff rotas to ensure all client visits are covered effectively
Act as a main point of contact for clients, staff, and external professionals
Support with recruitment, onboarding, and ongoing staff development
Maintain accurate care records, update care plans, and assist with risk assessments
Respond to queries and resolve issues in a professional and timely manner
Participate in the on-call rota, with additional pay
Assist the manager in ensuring the service meets CQC standards and regulations
